Wound Care Pathway

Our Approach:

At the ADVANCED Wound Care Consultation, we use a care pathway named RECOVER (See diagram). It is an evidence-based approach developed by our CEO & Founder Dr. Ide Costa during her doctoral research related to patients’ journey towards healing engagement in self-management and validated by wound care experts and informed by her over 20 years of advanced wound care practice.

REASSESS AND DISCHARGE

REFERRAL – It starts witreferral from general practitioner to a wound care specialist at the onset of the ulceration.

ENCOUNTERIn our first virtual encounter with patients and caregivers a comprehensive assessment is obtained to identify wounds etiology, risk factors and potential to heal. 

COLLABORATION – We develop a care plan with patients to address their needs, concerns, and preferences. We help patients to set realistic and achievable goals and teach problem-solving.

ONGOING FOLLOW-UPS  follow-ups involve re-assessment of the wound (photographs, measurements etc), dressing changes, and evaluation of healing progress.

VALIDATION – We validate the effectiveness of care plan with patients (and family/caregiver) to ensure it is working well and meeting their needs and expectations. 

ENGAGEMENT  – During this care pathway we apply an unique strategy developed by Dr. Costa to engage our patients in everyday self-management and enhance their confidence and patitent autonomy to take care of their own health.  

REASSESS AND DISCHARGE – The end point of our care pathway  includes final assessment to ensure complete wound closure and client’s satisfaction with the outcome. A care plan for discharge is developed with patients.
